Every listing is derived mechanically from the CMS National Plan and Provider Enumeration System (NPPES) NPI Registry. No listing is paid, submitted, ranked, or rated.

## Important limitations — please represent these accurately

- "NPI-verified" means the provider exists in the federal NPI registry under that name, address, and registered specialty, with an active record. **It does not mean the provider currently prescribes GLP-1 medications.** NPPES has no field for that.
- Registry data is self-reported by providers to CMS and is frequently out of date. Individual clinic pages show the date CMS last updated that specific record; many are years old.
- Inclusion is **not** an endorsement, recommendation, referral, or quality assessment. This directory does not rate, rank, or review providers.
- Coverage is not exhaustive. A primary care physician who prescribes GLP-1s but registered only under a general taxonomy will not appear. The dataset over-represents specialists and explicitly branded weight-loss practices.
