There is no contact form on this site — we do not collect information through forms. Please do not include personal health information in your message.

Report an inaccurate listing

This is the most useful thing you can send us. Registry data goes stale, practices move, and clinics change what they offer. We correct or remove inaccurate listings at no charge and without conditions.

Include if you can:

  • The NPI number shown on the listing, or a link to the page
  • What is wrong — moved, closed, renamed, wrong phone, does not offer GLP-1 care
  • The correct information, if you know it

We act on correction reports within a few business days. Because the directory is rebuilt from the federal dataset on a periodic schedule, the change becomes visible on the site at the next data refresh.

If you are the listed provider

You will never be asked to pay for a correction, a removal, or a better position. We do not sell placement. See our about page for the full advertising disclosure.

Corrections. You do not need to prove ownership to have a factual error fixed. Send us the correction and we will make it.

Removal. If you would prefer not to be listed, say so and we will remove the listing. We do not require a reason. We do confirm that a removal request comes from the provider it describes — a reply from an email address at the practice’s domain, or a call back to the phone number on the federal record, is enough. This is only to stop someone from removing a competitor’s listing.

Note on your federal record: we can only change what appears on this site. Your underlying NPPES record is maintained by you with CMS — to correct it at the source, go through NPPES directly. Our next data refresh will pick up the change automatically.

What we cannot help with

  • Medical questions. We cannot advise on whether a GLP-1 medication is right for you, dosing, or side effects. Ask a licensed provider — see our medical disclaimer.
  • Appointments. We are not affiliated with the clinics listed here and cannot book, reschedule, or contact them for you. Call the practice directly.
  • Prescriptions, refills, or records for any listed clinic.
